Instructions

This Incident Report Form is intended to document any accidents, injuries, misconduct, or unusual incidents that occur during hockey-related activities, including practices, games, training sessions, and club events. The purpose of this form is to ensure accurate reporting, support follow-up actions, and maintain a safe environment for all players, coaches, officials, and spectators.  


Please provide detailed and factual information about the incident, including the date, time, location, individuals involved, and a clear description of what occurred. If applicable, include any immediate actions taken, witnesses present, and any medical attention provided.  


All reports are treated with confidentiality and will be reviewed by the club’s management or safety committee. Timely and accurate reporting helps us improve safety measures and prevent future incidents.
